High gear limit not adjusted
Tighten high limit screw to bring the top pulley slightly inward
Low gear limit not adjusted
Derailleur or hanger are bent
Tighten low limit screw to bring the top pulley slightly outward
Straighten or replace items
Top pulley is too far from the sprockets
Top pulley is too close to the sprockets
Cable is too tight or is binding.
Withdraw the ‘B’-screw to narrow the gap
Tighten the ‘B’-screw to widen the gap
Turn barrel adjuster clockwise and lubricate and check cable for damage
